I was trying yesterday first 4 axis machining and things goes wrong, I checked the g-code online and it looks good there. Please see attached photo and g-code file.

I have MK1 controller from 2011 and never tested it on true 4 axis machining.
Also X Y and Z colors are wrong in software, and the tool path is reversed on Y axis that is my rotary axis on.

Apart from G40.1 g-code (which is used for C axes on some Fanucs) .and incorrect toolchange in beginning I don't see anything wrong. NCviewer displays this incorrectly
I removed what was wrong for you, the problem is the deviation from the tool path marked in red on the picture, I ran this code and it broke my part exactly as it looks in CNC USB Controller, but in NCViewer tool path it is exactly as it should be. I have attached the modified tool path to check it.
Second, in all CAM CAD programs the X axis is red and the Y axis is green.
Third, the tool path does not look the same in the nc viewer with CNC USB Controller as an orientation.

Found the problem, B axis in machine definition was on Y+, changed that to Y- as it is on my machine placed, tool path is good, orientation also good, but god still X axis must be RED and Y axis GREEN.

Axes colors are "standard".

Exactly as I said X RED and Y GREEN, in CNC USB Controller software the X is green and Y red as you can see in pictures bellow.
Seems that old software used these color. I never noticed they are not correct.
Why don't you use TNG?
