Not knowing how much current your 5v break out board needs, it make it hard to answer. If it not very much I would use a wall plug that has a power supply that puts out 5V.

Can you not get if from one of the 4 pin plugs on the PC P.S.?
What is the 5v supplying? If you have a power transformer in the system you could wind around a few turns on, 10 turns should do it with a bridge rectifier and cap.
There is also the cheap wall-wart type, these are now regulated supplies.

He needs a level shifter from what I understand. OP has a 24v proximity switch that has a pnp output. Use a resistor network...6.2k & 1.6k.
