Sorry, I have a MX4660 and just assumed the MX3660 was the same as the MX4660 with one less stepper output. Looks like you will need and additional 12v power supply.

You don't need an additional power supply.
Brown wire from proximity = +12v out 0n mx3660
Black trigger wire from proximity = input 1 0n mx3660
Blue wire from proximity = ground 0n mx3660...

Thanks for the reply guys. I contacted Leadshine and they sent me the diagram for the 86HS45D motors. I have included it in this post for those interested.
Did you figure this out? I have the exact same problem as you. My motors only have four wires red, blue, yellow, black and Leadshine's wiring diagram only shows an eight wire hook-up.
