Look in the axis control in one of your tool paths. if you click on Axis control you should see on the right top, 4-axis with 5-axis under it. If you don't see that you need to get your post fixed....

I would try the sheets op. first also. Sounds like you just don't have enough machining suface. Using sheets will fix that issue.
That sounds just like the G68.2 on a Makino HMC. Your mdd and control file's is probably difined a little wrong. It could be your post also. Unless your doing some true 5 axis programming you can...
Yes, I had that issue. I went into the configuration and had to set it to open Solid Models in there. No more issues.
I would start with the feed rate. The feed can mess with the size and shape. Or use cutter comp and add a different D for each hole diameter.
