I was referring the power supply that mounted on Fanuc 0M control, the part number would be something like that A16B-1212-****, not A06B-6087 which is servo/spindle power...

Sounds like the WGoyer advice is ignored. It would be a power supply (on Fanuc 0 Control) issue or power supply fuse pop.

al-04 means The input power supply is abnormal (open phase).

I just found 6079 and 6096 have a same base module board, it just have different interface, have a look on attached pdf page. There should be something to communicate 6079 with Fanuc 21i control.
