What might be the reason of this CNC milling Failure

We heard a sound, that see this defect on our part. The working tool is 5 carbide inserts, Ø50mm side cutter (Face mill)
    depth of cut:2mm
    Feed is variable arround 400mm/min-2000mm/min
    Spindle Speed = 4200 rev/min
I run the same nc code yesterday and everything was fine.
I wonder the reason of this failure on this image attached to the post.
Chip stuck, unstable stock or bad machine
.The code continued a bit after this failure just fine.

    Photo of cutter

Here it is. What I inspect is that the inserts seems fine.
Your saying is very logical to me,but our operator says that if that was the case the insert would break.

No the insert would not break,

The inserts may have got dull and the material could then heat up and pull up into the cutter how thick is that material, how much experience does the person doing the job have

That is not a good cutter to be using for a job like that

    I understand.
The material is Cu-ETP (very small amount of other contents, can be consider as pure copper) The depth of cut ap was 2mm : With stepover of %12 (ae=50mm*0.12=6mm)
Operator is still young,he has almost 9 years of experience but in a limited range ( mostly holes and breaking chamfers on 6-15mm materials.) I am in my first year.We are still improving.
We don't have large range of cutting tool in our closet, looking forward to get a decent stack.

So thank you very much for the explanation. I think I understood.
