Can anyone tell me how to put a 4th axis on a EZ TRAK DXII. I see the C and S Axis in the PFM file under tune. And I have an extra Drive card and servo . But I do not have software for the control;

The software will not support it. Do you need a true 4th axis or will a programmable indexer do?

Need a true 4th axis .
The EZ Trak software was never designed to read a 4th axis. The C and S axis were basically setup as spindle and some sort of like air/coolant system. If you wanted a 4th axis readable, you would have to use a seperate DRO.
