HELP NEEDED - Facing off gone bad!!!

Any ideas what whould cause the finish shown in the attached picture.

It has started happening with every piece we cut. We use our lathe for refinishing alloy wheels and this is the finish we get on every wheel we skim. The set up piece in the picture visually highlights the problem beter than on a wheel. This was simply manually cut, jogging in on the X axis.

Any help would be appreciated...its driving me mad

Have you tried a different tool or checked the head stock bearings and made sure you got plenty of clearance on the face of tool so the swarf doesn't catch or build up also is it chipping and you are using ally tips in your tool
