Followed your link, but its late, and I didn't feel like reading through 60 pages of gibberish to try and find out what your problem is. Try again.

Have a look at pages 16-17 for the actual failure....
Started on 16 and scanned through to page 22 and didn't see anything. You probably have your posts per page set higher that a guest would be, so page numbers aren't much help.

Double check your wiring first. My first wild guess would be that you exceeded the 28V on the supply end. If the measured voltage is near 28V, did it have any significant ripple in the output that would push the spikes higher? Rich
