    Error 416 X-Axis Disconnect

We are having issues with one of our older O-MC controllers. I will outline the history of the problems and the actions that have been taken so far.
The controller is using three axis X,Y and Z. Currently the X-axis is giving us a 416 X-Axis Disconnect.
Since we have four (4) similar machines I am able to interchange parts to try to isolate the problem.
Currently I have interchanged the following items one at a time to try to eliminate the problem.

    1) X-Motor
    2) Cable from Motor Encoder to Axis Board (Servo Feedback Cable)
    3) Cable from Axis Board to Servo Amplifier.
    4) Servo Amplifier
    5) A16B-2200-0360/04B 1/4 axis board.
    6) A16B 1211-0901/12 B PMC-M board.
    7) All parameters have been verified with adjacent machine.

On occasion I have been able to get the controller to power up, but as soon as there is a certain vibration it will go into X-Axis Disconnect or if I disconnect the encoder cable and re-seat it I will sometimes get it work momentarily. I would like to know what actually causes the X-Axis Disconnect? and what else can I check/replace.

    Thanx in advance

There is a chance that the base at the mother board is bad(cold weld), try changing the whole controller: mohter board, memory, etc.
