I just set the lathe to cut 11.5 threads per inch and kept going deeper till the mating part screwed on, and called it close enough.

I agree with Al.
The HP, watts and Amps indicate 220V.
I think that the 22nV is really just a poorly placed"0" as in 220V.
The 14g wire and plug with 2 flat contacts will be fine.

If your motor has 3 wires comming out and one of them is not green, it is possible that you have a 3 phase motor.
often times the ground connection is not a wire, but instead a screw terminal....

I checked with Dan, His was a refit of a mill that was CNC in an earlier life.

It looks like my only big issue will be the Z axis.
There are no exposed parts of the quill till it is about 1 inch...

It currently has the worm gear on it, it looks to be just a roll pin holding it.

The machine does not now have ball screws, but I do intend to add them. The x-axis screw is fairly worn and there is almost 1/4 turn of backlash right now.
