Update as the issue is resolved. The error was the encoder brass wheel not aligned with the 6 light holes (they line up with the 6 light sensors on the PCB) from the lamp so just needed a small...

Is there any update on how this issue was resolved? I have the same issue after cleaning the taco commutator and re-assemling the motor.
