4- 40, 40 revolutions for 1 inch of feed so a speed of 40 rpm and 1 inch per minute feed would be right for the tap.
But your machine does not run at 40 rpm, so multiply both...

Line voltage will vary, a 240 volt rated fan will work at 220 or 230 volts.
A 240 volt fan would not last long running on 110-120V.
The fuse should be just a little higher that the rated amperage...
Like popspipes and UMAN say, I would be checking for loose connections and input power voltage.
It could be low voltage on just one leg of your power.
Does it do that with new sharp tools?
