Ok, back in the office. Looks like quite the debate on which terminals to use for single phase input. I double checked the manual for this SK2201 and still find NOTHING about connecting to single phase. I'm attaching the cover page and wiring page for size 2.

It does seem very strange to me that this drive would be capable of running on single phase and that they wouldn't mention it in the litterature. That's such a huge selling point, especially since it appears to be such a rare capability at this power level. So I guess my next question is, if it's not capable of running on single phase and I power it up that way, what damage could I do?

First step is to get the thing up and running...I will do the cap reforming procedure as soon as I get my hands on my buddies variac.
