Fadal VMC15XT Servo amplifier fault on all axis

This machine is 1997 Fadal VMC15XT with 88HS control

When doing CS after powerup i get

    X axis servo amplifier fault
    Y axis servo amplifier fault
    Z axis servo amplifier fault

    Error number is #18

Pressing JOG to reset e-stop results Amplifier fault on axis that is selected with knob.

What i have done so far:
    - Measured all fuses and breakers -> none of them was blown
    - Measured +12/+5/-5V power supply -> voltages are good
    - Reseated every card/connector/relay

What would be next thing to try?

Check the voltage at the blue cap. It should read 125 to 165 volts DC. If it does not then there may be an issue with the blue cap or the amp chassis.

There is about 11.70VDC over blue cap.

120VAC can be found from the other side of large solid state relay next to bridge rectifier which should provide this +170V

If you have 120vac into the bridge rectifier but low DC voltage on the output side then you have found your issue.

After first power on, look at the amp boards and see if there are any lights. (you may have to have someone else push the green button while monitoring the amp boards.)
If amps light then go out after 3-5 seconds, you may have an issue with the resolver circuit.
If all amps stay on until you try to CS the machine, I would instead try jogging one axis at a time BEFORE trying to CS.
While thinking about it, did you make sure the E-stop button is released? Sometimes the most simple solutions are overlooked.
