Double check focus height, and try to change the velocity. Covering the glass with wet paper could also be a solution.

You need to use a low resolution (dpi) for glass to look good, the image is created by micro fractures so if you don't have sufficient spacing between points it will look blurred.

A damp paper towel on the surface can help too as it reduces the fracture size which allows a higher resolution image.

The type of glass is also a factor, cheap glass seems to work better. Avoid any crystal or thermal glass as it will shatter.

When we test the materials, we often set the DPI of the image 600-1200

If you're engraving something other than a photograph, go with sand etching, for less than $100 you can go to harbor freight and get a air eraser, some aluminum oxide media, and a pressure regulator. The sand etching will look much nicer than laser. You can use sign vinyl as your masking.

How do you cut the sign vinyl? Do you use a knife cutter, or can this stuff be lasered too?

I'm a little wary on account of the fumes hazard for the optics. PVC is a no-no, how about thin vinyl sheet?

I use a knife cutter, I run a sign biz but I just use a cheap $200 chinese vinyl cutter for these jobs. If you do some research there are polyester based vinyls out there instead of PVC based. The polyester ones are laser safe. I couldn't tell you which ones those are, but I'm sure if you ask around someone will know.

You probably won't like the look of lasered vinyl either I forgot to mention it melts the vinyl pretty bad. You'd be better off looking for a cheap vinyl cutter, I've seen some 24" ones on ebay for less than $200.
