Parker SM232AE BLDC Servo - Melted Down

Can anyone shed some light as to why my servo motor melted down last night?
I am running Galil DMC-1880, with Mach3 as frontend. Normally after a job, I park the spindle and hit reset in Mach 3. This disables the amps and I go about cleaning things up. Last night I forgot to hit reset and after 5 hours of sitting idle (but motors enabled) The motors on X were HOT, to the point the heat travelled down the ballscrew, 24" or so. The slaved motor is now frozen and will not turn, either with power or without power.
I have a fairly tight tuning, without oscillation (for the most part). Occasionally after I jog a motor may hum until I tap the jog and then the humming stops.

    Thoughts?

How high is your supply voltage, is it in the normal range for the motors ?
