wiring reversing switch for a 220 v motor

i've got a leeson motor with an L1, L2 and T5 and T8 taps. there are also 3 terminals on the motor 1,2,4. L1 is connected to 1 and L2 is connected to 4. T5 and T8 switch between 2 and 4 for opposite direction of rotation. can anyone give me a schematic on how to wire this up? i also have a 6 contact rotary switch that cam on my IH mill that i want to use for this. or give me another switch part number and i'll go and buy it.

    thanks
    Jerry

Best way to do this is with a DPDT switch.
2 and 4 go to the common center terminals.
T5 and T8 form a X to the outer terminals
