How is the colour done ? I am guessing it is painted on after the litho is carved...?

Even easier than that...I used HP color transparency film.
1st I converted the full color image into a CMYK image and then deleted the "K" channel which is the black or shades of gray that the lithophane already represents.
Then glue the transparency to the back of the lithophane being sure to line it up just right.
Pretty simple...Huh?

I wonder how the color image would look If you printed 2 sheets, one of them CMY & the other only the k?
Then sandwhich a piece of clear plexiglass between the two HP films, with a backlight.
Do you think it might give it a 3D type look, or maybe look like the image is moving as you walk by the image?
