Well I finally found the time to re-wire the power supply. I took it back to what I think is the factory original. Let me know if you see something wrong.

You only appear to have the incoming power connected.
Although it would power up as you have it, the white and black should be reversed so the black feeds the switch/breaker instead of the neutral, also connect the green wire to ground terminal.

The white and the black wire switch is easy enough. The reason I don't have out going power is because the motor this thing will drive, is over in storage. I need to go get it today and try this out. I did attach the ground because this was just a short test cord. Thanks for the input. I'll let you know how things work out.
