I have on occassion, taken a mill that was half the thread count (i.e. 20 tpi) and made two passes. Not horribly efficient, but functional for a few parts.

If you have a single toothed thread cutter, you can have any tpi with it, though the process would be less efficient.
OD or ID thread and what size.
If it is OD you can cut just about any size using a lathe threading tool in the spindle, but as mentioned it is not efficient.
You can also mill internal threads using a latge tool but then your ID has to be larger than about 1" because the tip sweeps out a circle larger than the bar it is mounted on.
An open mind is a virtue...so long as all the common sense has not leaked out.
