Don't know if anyone else is offering these services but we can 3d laser scan your objects and give you a STL or cad drawing.
Prices start at $100 per scan not including shipping. Located in Las Vegas.
We can print out with FDM 3d printer and CNC on a Haas VF4 too.

What is the scan envelope? What kind of cad model....nurbs or mesh?

Basic Envelope is 10"x10"x10" but we can do any size with multiple scans.
We can supply raw data points, STL nurbs and CAD depending what you need.

Well we could xray the object and digitize the internal surfaces or use a CAT or even a MRI/nuclear resonance type scanner.
Any project is possible if you have the money to pay for the time and research involved.
Once the points are taken, we can build the model.
