It's a lathe with a spindle, but it works the same way though.

A week or so from now I'll have a Fadal VMC with a full fourth. Can't promise I'll be running programs by then but I'm working on it. I'll post pics when I get everything installed.
As far as CAD/CAM goes I just got Mastercam mill3, all of their levels come with multiaxis capability, and learning 4 axis is going pretty smooth so far.
You can index, do a 4th axis wrap, or run as a true 4th axis.
