    Limits and logic high/low

I have an IDC Danaher motor controller that uses logic input for shutdown, logic high for enable, logic low for disable. I would like to use this for EOT. It has 2 terminals marked with a +and-. The controller operates with no wires landed on terminal. http://www.idcmotion.com/support/Manuals/6001v17.pdf
On page 6 of the link shows what info I have. How do I hook this up, I have 5,12,and 24volt supplies available for the logic supply and n/o and n/c switch contacts. If anyone can give me some tips they would be appreciated.
    Thanks, Steve
