I have made my own up, I suspect it may be easier/cheaper to make your own.
I used small toggle switches that had centre off, one way was momentary to simulate P.B.'s and the other maintained to simulate function switch if needed.
You did not specify what kind of output you require, I am assuming you want 5v in and 24vdc out?
If so a ULN2803 or similar should work.
For the ±10vdc I have used a battery box, but to integrate it into a unit there are voltage converters on one chip such as the ICL7662 that will give you ±vdc and use a 5k~10k pot. to give you variable analogue.
I would be surprised if there is an off-the-shelf unit exactly like that.
