M41SxxxM3 worked great, also M42SxxxM3 worked as well, I assume ,M41 is high gear, maxed out machine at around 3700 RPM, M42 is low speed, RPM gauge reads whatever I put.
I noticed some noise coming from the very L/S of the spindle, pulled the covers and found the bearing for the bracket that holds the chuck limit switches is shot. Looks pretty simple to replace, is it? Where is the best place to get parts for this machine, an Okuma dealer or are there other sources?

Glad you have it up and running! I would call Okuma for a price on the bearings but chances are you can get a better deal through a bearing house.
How are your belts? Just curious as mine were quite bad. They would slip under accel and decel. Bad belts can also cause noise.

It depends on the bearing in question and where it is located. Is it inside the hydraulic actuator? That would be the large aluminum housing that the "dog house" the limit switches sit over. If they are in that, that is normally an assembly purchased directly from Kitagawa. They can sell you a full rebuild kit, as if you're opening the actuator up to replace a bearing you'll disturb the seals which means you should replace them.
If this is not the bearing, then because it is outside the headstock itself, it will most likely be a 6 series bearing with or without shielding (IE 6206 or 6206zz). These can be picked up for a very low cost at any local bearing house.

Hi guys,
I am having trouble with our LB15 with OSP5000L-G. It worked tirelessly for more than 15years without any trouble but last couple of days we are having trouble.
It loads up to
LAME6-05-I.POL
LBA06-05-J.POL
LBME6-05-J.POL
LAZ06-05-I-AP01A.POL
And then goes to the error screen
DC power supply to servo amp is indicating ok but on the both servo amps red LV led is on.
I tested all the fuses in these boards and are ok.
Can anyone help?

Power supply sequence is not executed until successful boot, it's normal.
Bubble memory problem ... Is the software genuine? Was machine moved or some other occasion to squeeze some cable?
